Quality post-acute care is crucial for patient recovery. With the number of patients requiring post-acute care in an environment with reduced options, and the resulting bottlenecks in hospitals due to difficulty of finding available beds for discharge, the “SNF at Home” model presents a valuable opportunity to expand post-acute care capacity. This integrated approach allows services to be delivered directly to patients in their own homes, promoting recovery in a familiar environment. This not only enhances patient resilience but also reduces caregiver stress. The panel will discuss the legislative and regulatory frameworks needed to implement and support this innovative care model.

The forecast of a leadership team and levels of leadership in an organizational chart can be difficult to develop. Knowing who and what to consider as resources/stakeholders is vital to help promote decisions to this important aspect of your program. This working group discussion will provide insight on how one program began this process. We will review three key points. 1) When to consider modifying your leader team and organizational chart review. 2) What key stakeholders to help assist internally and externally. 3) What methodology to utilize to help develop your organizational chart.
