Hospital at Home has been proven to deliver better clinical outcomes, generate enhanced patient satisfaction, and reduce costs. But despite this potential, US Hospital-at-Home programs have not fulfilled their promise with most programs failing to scale. In fact, CMS reported outcomes showed that in 2023: less than 9% of waiver-approved hospitals accounted for > 70% of all US admissions (which represented only 0.01% of Medicare admissions across all programs); nearly two-thirds of hospitals had zero admissions; and of the 109 hospitals reporting volume, the average weekly admissions was < 2 patients per hospital. Join us to discuss lessons learned from Hospital-at-Home leaders on how to transform your program and overcome program growth and quality challenges. • Develop a strategic approach to patient eligibility and payer expansion that will improve ADC and enable scaling • Review tactics to achieve a > 65% patient acceptance using “the art of consent” and simplified workflow logistics
