This session will review the current evidence around Hospital at Home in the context of Medicare policy. It will then to turn to how Medicare could cover Hospital at Home in terms of benefit design and payment. Finally, the session will conclude with a discussion of what potential evidence is needed to influence Medicare coverage decisions.

Australia holds the longest experience in the establishment and delivery of Hospital at Home in the Anglophone world. Within this experience there may be lessons for the development of HaH in similar health systems starting the HaH involvement. This session will explore the progress of Hospital in the Home since its inception in Australia in 1994, based on both research and personal experience. It will describe the major points of policy and practice inflection over that time, and their possible implications including a description of the current scale of the HaH service intervention in Australia. UMass Memorial Medical Center (UMMMC) is the primary safety net tertiary care hospital in central Massachusetts. Having experienced multi-year challenges with inpatient capacity limitations and emergency department boarding times that were further exacerbated by the COVID pandemic, UMMMC began development planning for its Hospital at Home (HAH) program in January 2021 and launched its HAH program in August 2021. In April 2024 the program admitted it’s 2000th patient with strong clinical outcomes. Here, we describe the UMMMC HAH program in brief and share our nearly three-year journey to early scale, focusing on our HAH program’s approach to early census growth, patient outcomes and health equity assurance.
