Differences in Clinical Practice in Hospital at Home

The goal of hospital-at-home care is to approximate that of the traditional bricks-and-mortar hospital. Hospital at home, though, is a complex model to coordinate logistically and aims to provide a more patient-centered delivery. In order to scale hospital-at-home programs safely due to factors related to delivering care in the home or factors that drive an improved patient experience, hospital at home requires programs to evaluate certain situations and approach them differently. By re-evaluating these situations, hospital at home can provide a more positive care experience, reducing over-utilization, while maintaining high quality care. This interactive session will cover case-based scenarios where there are unique situations in hospital at home and the considerations programs must need in operation of their hospital-at-home units. Examples that will be discussed include: timing of medications and infusions, lab testing and point-of-care use, deep vein thrombosis prophylaxis and glycemic management.
