Optional Pre-Conference Workshop 1A: Hospital at Home is Just the Beginning: How to Leverage Your H@H Capabilities to Advance Robust C@H Solutions and Improve Hospital Operations

The core capabilities required to support the H@H clinical operating model serve as a powerful catalyst for broader healthcare transformation. Providing acute-level care to patients in their homes necessitates orchestration of a variety of clinical and ancillary services, logistics management and last-mile delivery, technology-enabled remote patient monitoring, multi-channel communications, and robust command center capabilities, among others. These same capabilities are extensible to a variety of other use cases across the care continuum—not only for geographically-distributed and in-home care models, but also to help address today’s most pressing in-facility operational challenges through a seamlessly integrated clinical command center. Join us to explore the myriad of innovative opportunities that can leverage a foundational H@H platform, including the discrete patient populations they serve, their associated value potential, and success requirements.
