Dr. Malta is a Pharmacotherapy Specialist with Hospital at Home (HAH) at Tampa General Hospital. Dr. Malta completed her Doctor of Pharmacy degree at the University of Florida and completed a pharmacy practice residency at Tampa General Hospital. She has worked as a specialist of Transitions of Care and Geriatrics in Tampa since 2018. Dr Malta provides pharmacy services to HAH patients, including comprehensive medication reconciliation, therapy optimization, and counseling, and works to expand the HAH program. The HAH program has been shown to reduce readmissions at TGH, and in-home medication education is one driver of that success.
