Dr. Dan Davis serves in multiple medical leadership roles within Advocate Health. As Vice President and Physician Lead of Continuing Health, Dan has medical oversight responsibility for multiple home and community-based care programs within Advocate Health including Advocate Health Hospital at Home, Advanced Care at Home, Home Health, Hospice, Palliative Care and others. Under his leadership the Continuing Health strategies are being aligned and integrated to create a seamless care continuum experience for patients and providers in the ambulatory space. In leading Advocate Health Hospital at Home, Dan has been instrumental in building and scaling the largest Hospital at Home program in the nation supporting 16 waivered facilities in 4 major metropolitan areas across 3 states with an enterprise average daily census of 150 patients and caring for more than 26,500 patients since inception.

Dr. Sarah Schenck is a physician leader and board-certified primary care internist with a passion for improving how healthcare is delivered, coordinated, and experienced by patients and healthcare providers. She currently serves as National Medical Director, Specialty Care at Devoted Health, where she partners with clinical and operational teams to strengthen specialty care delivery for Medicare Advantage members. Her work focuses on building high-performing specialty networks, enhancing coordination between primary and specialty care, and advancing value-based care models that improve quality, access, and patient outcomes.

Alex Binder is a long-time member of the American Academy of Home Care Medicine (AAHCM), with more than 20 years of involvement in the Academy.
He has served as a board member for seven years and currently holds multiple leadership roles, including Treasurer and Vice Chair of the Academy’s Public Policy Committee.

Professionally, Alex serves as Vice President of the Parker Advanced Care Institute, overseeing operations that help patients with late-stage chronic illnesses stay home comfortably with the support of Visiting Physician Services and a palliative care team.

His responsibilities include enhancing and expanding the delivery of home-based primary care as well as broadening the reach of palliative medicine in hospital, post-acute, and community settings. Prior to joining the VNA Health Group, Alex was the Practice Manager for Visiting Physician Services for over 15 years.

Dr. Chiang served as the Medical Director for HomeCare Physicians (HCP), a suburban Chicago practice focused on delivering care to medically complex patients in their homes. The HCP house call program has made more than 142,000 house calls to more than 10,000 home-limited patients since its founding in 1997. Personally, Dr. Chiang has made over 38,000 house calls to more than 3,900 patients in his 24-year career. His practice and work have been featured on ABC News Chicago and in northeast Illinois’ Daily Herald. In 2014, Northwestern Medicine Central DuPage Hospital named him as one of 50 Unsung Heroes. In October 2019, Dr. Chiang received the “House Call Doctor of the Year” award from the American Academy of Home Care Medicine. In 2024, Dr. Chiang received the Community Leadership Award from the Chicago Health Executive Forum.

Dr. DeCherrie has a professional passion for home based medical care. She is the Vice President for Clinical Strategy and Care Delivery at DispatchHealth, the nations largest provider and enabler of in home acute care. Previously she was the at Mount Sinai Health system for 18 years and was the Clinical Director of Mount Sinai at Home, the home based service line. Dr. DeCherrie is a Clinical Professor in the Brookdale Department of Geriatrics and Palliative Medicine at the Icahn School of Medicine at Mount Sinai. Dr. DeCherrie graduated from University of Chicago in 1995 with a major in History, Philosophy and Social Studies of Science and Medicine (HiPSS) and then continued at Pritzker School of Medicine graduating in 1999. She completed her residency in Internal Medicine at Boston University where she was named chief resident.

Dr. Konstantinos “Kosta” Deligiannidis is the Medical Director of Northwell Health's House Calls program, a home-based primary and palliative care program within the largest health system in New York, and a participant in the Independence at Home demonstration program.

Eighteen years ago, he began his career as a full-spectrum family physician in a small-town health center in Central Massachusetts, caring for patients in inpatient, labor and delivery, nursing home, and outpatient settings. At UMass Medical School, he taught learners the benefits of home visits and inspired many to pursue this work. He was an inaugural member of UMass’s Quality Scholars program, where he developed a passion for quality improvement and system-based change. As a quality improvement director, he led projects presented at state and national conferences.

Katie Heimbichner, PMP, is a business development and sales leader with deep experience driving growth across healthcare and financial services. As Head of Business Development Operations at MedArrive, she leads sales strategy, market and industry research, and logistics planning to support scalable growth. She is known for cultivating and expanding multi-level partnerships across business lines to unlock new opportunities and deliver measurable outcomes.

Patricia Kentgen is a transformational healthcare executive with more than 20 years of experience building, scaling, and optimizing clinical operating companies. She has led organizations through high-growth phases, M&A integrations, and clinical innovation, consistently delivering improved patient outcomes alongside sustainable business expansion. Her 40+ year background as a professional registered nurse brings a strong clinical understanding of the healthcare ecosystem.

Most recently at Medically Home, she helped pioneer the healthcare-at-home model, forging strategic partnerships with leading health systems and payers to drive over $50M in growth and expand national clinical operations.

Dr. Leff is Professor of Medicine at the Johns Hopkins University School of Medicine. His research focuses on novel models of care delivery for older adults with an emphasis on home and community-based models of care, issues related to multi-morbidity and serious illness, risk prediction, and quality measurement and improvement. He Co-leads the HaH Users Group and the National Home-Based Primary Care Learning Network. He is Founding Executive Editor of the Journal of Advanced Home Medicine, past -President of the American Academy of Home Care Medicine, and currently serves as a Trustee of the ABIM Foundation.

Michael J. Maniaci, M.D. is the Virtual Health & Home-Based Care Clinical Operations Associate Vice President for Advocate Health. In this role, he oversees the strategic and operational development of virtual health and home-based care programs, driving innovation in high-acuity care delivery models that extend clinical services beyond traditional hospital settings.

Previously, Dr. Maniaci held multiple leadership roles at Mayo Clinic, including Enterprise Physician Lead for the Advanced Care at Home program, Medical Director of Consumer Digital Product in the Center for Digital Health, and Medical Director of Mayo Clinic Hospital in Florida. His other leadership positions included Chair of the Division of Hospital Internal Medicine and Associate Medical Director of the Mayo Clinic Multidisciplinary Simulation Center in Florida.

A physician executive with over 15 years of experience leading enterprise-scale clinical operations and utilization strategy within Evernorth Health Services, part of The Cigna Group. My work is focused on redefining how payers manage cost, quality, and utilization at scale—shifting from traditional, reactive utilization management to predictive, data-driven, and clinically validated models that deliver measurable financial and operational impact.

Dr. Melissa Morgan-Gouveia is the Director of Geriatric Education for the Department of Medicine at ChristianaCare. A geriatrician and clinician educator, her passion is improving the care of older adults through education. Her clinical practice is focused on home-based primary care. She currently serves as the Chair of the American Academy of Home Care Medicine Education Committee. She holds a part-time faculty appointment as an Assistant Professor of Medicine in the Division of Geriatric Medicine and Gerontology at the Johns Hopkins University School of Medicine.

Andrew W. Nance, MD, DipABLM is a family medicine physician, educator, and health system leader specializing in Lifestyle Medicine and whole-person care. He is the founder and Medical Director of Lifestyle Medicine at Advocate Health, where he designed and scaled a multidisciplinary department that integrates care across clinic, home, and community settings. Dr. Nance serves as faculty at Wake Forest University School of Medicine, where he teaches and conducts community-engaged research. His work focuses on redesigning preventive care delivery by building and implementing care models that apply behavior change principles to real-world clinical workflows across the healthcare continuum.

Bridget Nicholson, PhD, NP is a nurse practitioner and researcher whose career centers on enhancing care for individuals living with cancer. With expertise spanning community, academic, and virtual care settings, she brings a comprehensive approach to oncology care delivery. Dr. Nicholson has played a pivotal role in designing and implementing innovative virtual care models, including remote patient monitoring and digital symptom reporting systems that have advanced the responsiveness and personalization of oncology care. She recently completed a postdoctoral fellowship at the University of Utah focusing on rural health, where her research centered on optimizing oncology care delivery and creating more proactive, patient-centered care models.

Sarah Payne, DO, HMDC, is a physician leader in geriatrics, hospice, and palliative medicine, serving as a Physician Executive/National Medical Director at Gentiva. She is recognized for fostering person-centered care and received the 2021 Geriatrician of the Year award from the Arizona Geriatrics Society.

Rebecca has dedicated three decades to public health as both a practitioner and as a leader of care model design innovation, health system partnership and enhancing access to high quality care. Throughout her career, she has led transformative programs addressing the social determinants of health for at-risk populations, several of which have gained national attention. Under Rebecca’s leadership, Housecall Providers has expanded services to include a community-based palliative care program spanning seven Oregon counties, creating a full continuum of care alongside home-based primary care and hospice. She has also driven growth for the organization by partnering with payers across the state in value-based payment models that improve access, quality outcomes, and patient satisfaction.

Dawn Raska is a seasoned healthcare leader with over 25 years of experience in home health. As Director of Saint Francis Home Health, she has been instrumental in advancing patient-centered care, most notably through the development and implementation of the Skilled Alternative Program, which expands access to specialized services for diverse patient populations.

Jonathan Rhodes is co-founder of Daymark Health, a value-based oncology company redefining the cancer care experience for patients, providers, and health plans. Prior to starting Daymark, Jonathan was a Vice President at Healthcare Foundry, a platform to launch built-for-purpose, technology-enabled healthcare organizations. Jonathan's career has spanned sourcing, co-developing, investing, and integrating healthcare startups into large legacy health enterprises, such as Cerner, Cleveland Clinic, and UnitedHealth Group / Optum. Hank Ross is the founder of Physician Housecalls, a company that has provided more than 300,000 housecall visits in Oklahoma, Kansas, and Missouri.
He also founded and served as CEO of Ross Health Care, a home care company serving Oklahoma.

Mr. Ross spent fourteen years in hospital leadership and was a member of the Oklahoma National Guard, including a tour in the Gulf region during Operation Desert Storm. Most recently, he has been integrally involved in REACH High Needs ACO models serving some of the most expensive and frail patients in the Medicare system. He currently serves on ACO boards and is the point person for the Academy REACH HN ACO dedicated to housecall providers.

Smita Sawant, is a healthcare technology leader focused on lowering the cost of care, expanding patient access, and reducing administrative burden across the system. Over 20+ years, she has partnered with leading insurance payers and health systems to design and scale workflow-embedded solutions that measurably improve care quality, operational efficiency, and affordability. Recognized for bridging frontline clinical realities with scalable technology, Smita turns complex care delivery challenges into practical, high-impact outcomes for patients, clinicians, and health system leaders.

Dan Shields is the Chief Executive Officer of Digital Medicine at Ochsner Health. With over 20 years of experience in healthcare, Dan brings deep expertise in digital health strategy, innovation, go-to-market execution, and scalable care models.

A vocal advocate for transforming healthcare delivery through technology, Dan emphasizes collaboration among health systems, payers, and employers to drive better outcomes and reduce costs. Julie H. Silverstein, MD, FACP is a distinguished medical business executive and primary care general internist with a passion for creating and optimizing healthcare systems that deliver high-quality, equitable care. Dr. Silverstein has extensive expertise in value-based care delivery, clinical leadership, team development, and operational strategy.  As Chief Medical Officer at Oak Street Health (2022-2024), she led a team of over 900 providers, enhancing patient satisfaction, clinical quality, and provider wellness. Previously, as Executive Medical Director and then Division President of the Atlantic Division (2018-2022), she spearheaded the East Coast expansion of Oak Street Health, growing from 10 to 27 centers while overseeing all business and clinical operations. With over 25 years of experience in continuing medical education (CME) and health care quality improvement, Melissa Singleton has held senior staff positions at medical specialty societies and has also served as an executive consultant to health care organizations at national and state levels. She has a graduate degree in adult professional education/human resource development and brings extensive knowledge and expertise in educational design, research, quality measurement, and health care outreach.

In her role as chief operating officer at the Home Centered Care Institute, Ms. Singleton oversees the development, implementation, and evaluation of all educational programming, research initiatives, and a wide range of projects, services, and outreach aimed at expanding access to high-quality home-based primary care (HBPC) for patients who need it.

Jennifer Stanley is a dedicated healthcare professional with more than 25+ years of experience in clinical practice and healthcare leadership across the care continuum. She has led multidisciplinary teams in delivering high-quality, patient-centered care while driving innovation in care delivery models.

Jennifer is passionate about advancing hospital-level care in the home as a key component of the evolving healthcare continuum. Her work focuses on reducing avoidable hospitalizations, improving patient outcomes, and expanding access through safe, effective alternatives to traditional inpatient care.

Dr. Nathan Starr has led Intermountain's Hospital at home program since it's inception in March of 2020. In September 2020 he became Medical Director
of Home Services for Castell, over the Hospital at Home and Home Primary Care programs. Dr. Starr has led the Intermountain TeleHospitalist program
since November 2020.

Lisa is a nationally recognized expert on hospital at home law, policy and advocacy. During her more than two-decade career she has held numerous positions with CMS ranging from litigating attorney to member of the CMS-CDC COVID-19 Task Force. As a member of the task force, she helped draft the Hospital Without Walls waivers, and then co-led the team that wrote and implemented the CMS Acute Hospital Care at Home waiver. In addition, she also led the CDC/CMS nationwide nursing home strike force initiative to assist nursing homes that were experiencing large COVID-19 outbreaks. After leaving government in 2021, Lisa led the advocacy team at Medically Home, which was instrumental in getting Congress to extend the CMS Acute Hospital Care at Home waiver in 2022 and getting the House and Senate to introduce bills for a five-year extension of the waiver in 2024.
