Ila Mapp, DNP, RN, CENP, CCRN has been enhancing her nursing craft for over 20 years through travel nursing, bedside patient care, and leadership. In 2008, Ila joined the UNC Health organization advancing from clinical nurse to Nurse Manager. She has held a leadership roles within UNC Medical Center and UNC Health since 2013 with her specialty in critical care managing four specialty teams: Rapid Response, ICU Transport, Venous Access Team, and CVAD Liaison. Ila now serves as Director for Advanced Care at Home – an innovative utilization of technology providing acute care for patients in their homes. This model of healthcare incorporates the collaboration of internal and external community stakeholders inclusive of Paramedics and Home Health Nursing working together with the common goal of patient-centered care for the Triangle of North Carolina.
Jen has served as the Area VP for North Carolina Hospital at Home at Atrium Health since 2024. In her role supporting North Carolina, Jen has notably expanded both the census and service lines, developing the program to function like a hospital rather than just a single unit.
With 36 years of clinical experience in acute care, home health, and inpatient rehabilitation, and hospital leadershp, she has committed her professional career to evidence-based practice, patient advocacy, and lifelong learning for herself and others.
Jen graduated from Boston University in 1990 with a BS in Physical Therapy. She later earned a Master’s in Anatomy from Case Western in 1998, followed by a Doctorate in Health Science from the University of Indianapolis in 2021. Her dissertation and research focused on predicting discharge destinations after hospitalization. Jen is also passionate about teaching and frequently presents at conferences nationwide.
Dr. Eric Alper is SVP, Chief Quality Officer and Chief Clinical Informatics Officer at UMass Memorial Health Care and Professor of Medicine at UMass Chan Medical School.
In addition to his clinical work in Hospital Medicine for the last 25 years, Dr. Alper is a seasoned physician executive, having served as the primary physician champion for 2 large Epic implementations He has led many clinical and operational improvements. He has served as executive sponsor for UMass Memorial’s digital health strategy, Hospital at Home and other care at home strategies.

Latasia Belin, DNP, APRN, AGCNS-BC, ONC, FCNS is a Clinical Nurse Specialist with Atrium Health Hospital at Home and brings over 16 years of nursing experience with a clinical background in adult-geriatric care. Bailey Beauclair is the Operational Project Manager for Advanced Care at Home at UNC Health, where she partners with clinical, operational, and executive leaders to advance innovative models of care and support sustainable program growth. She leads strategic initiatives focused on expanding hospital-level care in the home, including program development, interdisciplinary collaboration, and clinical operations. Bailey has led more than 30 multidisciplinary projects aimed at improving care delivery, operational efficiency, and patient experience while helping grow UNC Health's Hospital at Home program. Prior to joining Advanced Care at Home, she served in quality improvement, organizational excellence, and hospital operations leadership roles across UNC Health, specializing in change management, Lean methodologies, and healthcare transformation. She holds a Master of Health Administration from the Medical University of South Carolina and is passionate about building collaborative cultures that foster innovation, engagement, and high-quality patient care.
Dr. Blind is an experienced nurse leader and emerging thought leader in care-at-home models, with a strong focus on advancing innovative, patient-centered healthcare delivery. As Regional Director of Home Services at NYU Langone Health, she oversees Education, Professional Development, and Quality Improvement across the Home Health Care, Acute Care at Home, and Hospice settings, driving excellence in care beyond traditional hospital settings. With a clinical foundation in critical care and a background in clinical informatics and quality, Dr. Blind brings a systems-level perspective to transforming how quality care is delivered. Dr. Robert Boxer is the Senior Medical Director of Mass General Brigham’s Home Hospital and has been working in home hospital leadership since 2019. Dr. Boxer’s interests and academic work have focused on interdisciplinary collaboration and healthcare innovation to promote patient care that is patient and family-centered, equitable, safe and high value. Dr. Boxer continues to work clinically in Mass General Brigham’s Home Hospital and as a traditional hospitalist at Brigham and Women’s Hospital and Brigham and Women’s Faulkner Hospital.
Michael Chin, MD, is an Associate Medical Director at Massachusetts Medicaid (MassHealth), and an Assistant Professor of Family Medicine & Community Health at the University of Massachusetts Chan Medical School. Michael’s areas of focus at MassHealth have included health equity, data analytics and clinical informatics. Michael is a board-certified physician in family medicine and clinical informatics, and his clinical work has included over 10 years as a primary care and urgent care physician at two federally qualified community health centers in Massachusetts.

Dr. Jared Conley MD PhD MPH is an Assistant Professor at Harvard Medical School and an emergency physician at Massachusetts General Hospital (MGH). He serves as the Director of the MGH Healthcare Transformation Lab, where he leads a team of clinicians and engineers to enhance the quality and affordability of acute healthcare through technology and innovation.
He additionally serves as an advisor to MGH’s Home Hospital program and as the Medical Advisor for the World Hospital at Home Community. His digital health work is broad but is particularly focused on new, tech-enabled models of care, including hospital at home (HaH). He was recently asked to chair the first-ever HaH technology forum in March 2023.
As the Chief Medical Officer of MassHealth, Jatin provides strategic, clinical, and policy leadership and oversight of all healthcare services for MassHealth members and represents the program on clinical matters. In his dual medical leadership roles, Jatin is the primary clinical liaison between MassHealth (Massachusetts Medicaid and CHIP) and For Health Consulting at U Mass Chan Medical School. As the Regional Physician Leader for Inpatient and Advanced Care at Home Care Experience across 21 hospitals in Kaiser Permanente Northern California (NCAL), Dr. Ashu Goyal provides strategic leadership focused on ensuring patients receive the highest level of care experience and clinical quality across hospital-based and home-based acute care settings. His expertise centers on care experience strategy, quality improvement, and physician leadership. Nolan Hoffer serves as Vice President of Operations for Home & Community-Based Services at St. Luke’s Health System, where he leads the scaling of Hospital at Home, virtual care, and post-acute services across the continuum.
With more than 35 years of executive leadership experience, Nolan has overseen operations across the full post-acute spectrum and has deep expertise in integrating care delivery models to improve access, quality, and patient experience. He has been instrumental in advancing innovative care models, including SNFist and Transitions of Care programs, and the development and expansion of St. Luke’s Hospital at Home.
Since joining St. Luke’s in 2014, he has played a key role in system integration, including the incorporation of Idaho Elks Rehabilitation Hospital, and now leads a diverse portfolio spanning therapy services, home health, hospice, and system chaplaincy.

Colleen Hole serves in an advisory role, helping guide Advocate Health’s home-based care strategy, which includes scaling Hospital at Home to all markets across the Enterprise, and working to advance legislative policy, payment models and corporate partnerships that support home-based care.
Previously, Colleen served in a dual role at Atrium Health as vice president of clinical integration and chief nurse executive for Atrium Health Medical Group, responsible for non-provider clinical practice and role optimization in ambulatory care. Abid Khan is a Senior Advisor at the Centers for Medicare & Medicaid Services (CMS) Innovation Center, specializing the design and implementation of multipayer models such as the Achieving Healthcare Efficiency through Accountable Design (AHEAD) and Maryland Total Cost of Care Models. He has a profound interest in transformational value-based care innovations including hospital global budgets, comprehensive primary care programs, behavioral and mental health integration, and hospital-at-home initiatives. Prior to CMS, Dr. Khan spent over a decade in leadership roles for the American Urological Association overseeing scientific programs spanning clinical quality improvement, quality measure development, patient safety, and clinical practice guidelines, and advised hospitals and health systems on quality improvement and cost-effectiveness. Abid's educational background includes a doctorate in health policy and management and a master’s degree in health sciences from Johns Hopkins Bloomberg School of Public Health, a master’s degree in public policy from the University of Maryland, Baltimore County, and a bachelor’s degree in biology from McGill University.
Thomas Kiesau is a Senior Partner with Chartis, the firm’s Chief Innovation Officer, and leader of the Digital Practice, the firm’s business unit dedicated to digital transformation planning and execution across the healthcare arena. Prior to assuming leadership of the Digital Practice, he led the firm’s Strategy Practice. With more than 20 years of experience, Mr. Kiesau has served as an advisor to many of the nation’s leading children’s hospitals, academic health centers, faculty practice groups, integrated health systems and community hospitals. He directs consulting engagements principally in the area of digital transformation, including digitally-enabled care model transformation, home as a site of care, digital consumer experience and digital operating model design and implementation.
Lisa Klosek, MHI, BSN, RN is the clinical leader and Nurse Manager for Atrium Health Levine Children’s Pediatric Hospital at Home program. With more than 30 years of nursing experience across ambulatory and inpatient settings, caring for both pediatric and adult patients, Lisa brings a deep and well-rounded clinical perspective to her work. She has been with Atrium Health since 2004, serving in a variety of roles that have shaped her passion for innovative care delivery.
Driven by this passion, Lisa earned her Master of Healthcare Innovation (MHI) and is currently completing her DNP in Nursing Leadership. During the COVID-19 pandemic, she led virtual care initiatives for pediatric patients, helping expand access and support families during a critical time. Lisa has since been instrumental in the development and launch of several new programs and now provides clinical and operational leadership as the Nurse Manager for Levine Children’s Pediatric Hospital at Home, which launched in February 2025, advancing a patient-centered model of hospital-level care in the home.
Dr David M Levine, MD MPH MA, is an internationally renowned expert in designing and implementing advanced home-based care, using digital health technology to drive health outcomes, and evaluating the quality, safety, and experience of health care. He is an Associate Professor of Medicine at Harvard Medical School, clinical director of research and development for Mass General Brigham Health Care at Home, and co-director of the Harvard General Internal Medicine Fellowship at Brigham and Women’s Hospital. He founded Brigham’s home hospital program and co-founded and co-chairs the Hospital at Home Users Group, a collaborative of home hospital programs throughout the US and Canada. He is core faculty at Ariadne Labs, a center for health systems innovation at the Harvard School of Public Health and Brigham.
Michael J. Maniaci, M.D. is the Virtual Health & Home-Based Care Clinical Operations Associate Vice President for Advocate Health. In this role, he oversees the strategic and operational development of virtual health and home-based care programs, driving innovation in high-acuity care delivery models that extend clinical services beyond traditional hospital settings.
Previously, Dr. Maniaci held multiple leadership roles at Mayo Clinic, including Enterprise Physician Lead for the Advanced Care at Home program, Medical Director of Consumer Digital Product in the Center for Digital Health, and Medical Director of Mayo Clinic Hospital in Florida. Danielle has been working in healthcare since 2009. She entered the Hospital at Home space in 2020, helping launch the Mayo Clinic Advanced Care at Home program, while employed at Medically Home. At Medically Home Danielle also worked with the Cleveland Clinic, Kaiser Permanente, and Covenant Health HaH programs. In 2024 she left Medically Home and joined Baptist Health based in Jacksonville Florida where she serves as the Operations Director responsible for building, launching, and overseeing Baptist Hospital at Home. She has her bachelor’s degree from Penn State and is currently finishing her MBA from Eastern University.
Dr. O’Shea is interested in applying his training in clinical medicine and business administration to build and refine systems of care for hospitalized adults and children. In addition to serving as a hospitalist, he works as a physician advisor to support clinical operations across UNC Hospitals.
Sarah Paez, MS, GNP-BC, is the Director of Case Management and Nurse Navigation for Mass General Brigham Home Hospital. A nurse leader with more than 20 years of experience in healthcare innovation and community-based care, she has played a key role in developing, launching, and operating multiple hospital at home programs across diverse health systems, including Mass General Brigham and South Shore Health.
Her expertise spans hospital at home, care management, nurse navigation, geriatric and palliative care, virtual care delivery, and healthcare operations. Throughout her career, Sarah has designed and scaled innovative care models that bring acute, primary, and palliative care into the community, helping health systems expand capacity, improve patient outcomes, and deliver care where patients most want to receive it. She is passionate about building patient-centered models of care and developing the programs, teams, and systems that enable patients and clinicians alike to focus on what matters most.

Evan Pankey, MMSc is a product leader and consultant with over 7 years a experience in Hospital at Home. He has supported over 20 health systems in technology, opersations and strategy. Evan is currently a consultant with Tripp Hollander Advisors.
Dr. Margaret Paulson is an Assistant Professor of Medicine at the Mayo Clinic College of Medicine and Science. She has experience in patient care across the spectrum of Internal Medicine. She currently practices as a rural hospitalist in the Mayo Clinic Health System in the Northwest Wisconsin (NWWI) region and leads the Home Health team. In 2019, she became the Medical Director for NWWI in the exciting establishment of Mayo Clinic’s Advanced Care at Home (ACH) program, an innovative platform offering hospital-level care in the home. The launch of ACH allowed Dr. Paulson to call upon her experience in outpatient and inpatient Internal Medicine to expand high quality telemedicine in transformational ways for Mayo Clinic.
Holly Pearson, RN, MHA, is a healthcare executive based in Omaha, Nebraska, with a focus on improving patient outcomes while challenging traditional care models. She brings extensive experience in aligning clinical workflows, technology, and population health initiatives to enhance access, engagement, and care delivery across the continuum. Holly is passionate about creating practical, patient-centered solutions that meet people where they are, leveraging data, digital tools, and interdisciplinary teams to support more responsive care while maintaining financial responsibility in today’s healthcare environment and supporting its long-term sustainability.
Vance Purcell, MBA, BSN, RN, PHN, is the Kaiser Permanente Northern California Advanced Care at Home's Clinical Nursing Director, leading Clinical Operations for the growing service line. He joined the department in December 2023 after spending nearly 10 years leading one of KP's largest Home Health and Hospice programs in Sacramento, California. Since joining the ACAH team, he designed the nursing infrastructure and worked with all vendor partners to grow and develop the service, including designing and building the nursing leadership structure from the ground up and designing the care transitions team responsible for service line throughput. Additionally, he leads key initiatives in Care Experience, Performance Improvement, and Expansion Training.
Yair Saperstein, MD MPH is a dual board-certified physician in internal medicine and clinical informatics and is CEO and cofounder of Avo, an EHR-integrated platform for AI. He is a member of Alpha Omega Alpha (AOA) and is affiliated with Mount Sinai Hospital.
Dr. Saperstein served on the implementation team for multiple Epic go-lives, and was inpatient medicine Chief Resident at HHC's Kings County Hospital, leading over 400 providers through New York's first wave of COVID. Dr. Saperstein graduated from Albert Einstein College of Medicine with distinction in research in global health and from SUNY Downstate with a Masters in Public Health in hospital policy and management.
Previously, Dr. Saperstein cofounded and led two non-profit organizations for over ten years, StartScience.org and Teach4Kids.org, scaling both internationally. Additionally, he is an acclaimed classical concert pianist and has won numerous awards, including The Jewish Week's "36 under 36" most influential Jewish Americans.
Smita Sawant, is a healthcare technology leader focused on lowering the cost of care, expanding patient access, and reducing administrative burden across the system. Over 20+ years, she has partnered with leading insurance payers and health systems to design and scale workflow-embedded solutions that measurably improve care quality, operational efficiency, and affordability. Recognized for bridging frontline clinical realities with scalable technology, Smita turns complex care delivery challenges into practical, high-impact outcomes for patients, clinicians, and health system leaders.
AnnMarie Short, MSN, RN, PCCN, NE‑BC is an experienced nurse leader with 28 years at NYU Langone Health. She holds a Master of Science in Nursing (MSN) and is board certified as a Nurse Executive (NE‑BC). Her background includes 28 years in medical‑surgical and telemetry nursing and more than 23 years in nursing leadership.
Since joining NYU Langone’s Home Hospital program in September 2022, AnnMarie has served as Nurse Manager, providing daily operational oversight for the delivery of acute‑level care in the home. She has been instrumental in the development and implementation of Hospital at Home workflows, helping to operationalize safe, scalable, and high‑quality care outside the traditional inpatient setting. AnnMarie leads interdisciplinary collaboration, drives nursing quality initiatives, and supports innovation that strengthens clinical excellence, patient experience.
Eliza “Pippa” Shulman, DO, MPH, is one of the world’s foremost authorities on providing advanced care in the home. She joined Dispatch Health’s leadership team during the company’s merger with Medically Home, where she served as Chief Medical Officer and Chief Strategy Officer. As the clinical architect behind many of Medically Home’s most innovative models, Dr. Shulman helped shape what is now part of Dispatch Health’s approach to high-acuity care in the home.
Ms. Smoak has been with the Agency for Health Care Administration since August 1995. She is the Deputy Secretary of the Health Care Policy and Oversight Division. The Division licenses and/or certifies and regulates 30 different providers, including hospitals, nursing homes, assisted living facilities, and home health agencies. In total, the Division licenses, certifies, regulates, or provides exemptions for more than 50,000 providers.
Ms. Smoak is also the State Survey Agency Director for the State of Florida and holds a Master of Science in Health with the program emphasis in Aging Studies. She is also a Qualified Intellectual Disability Professional (QIDP). In 2009 and 2017, she received Survey & Certification Achievement Awards, presented by the Centers for Medicare & Medicaid Services for her work in Survey and Certification.
Kenneth Sng is co-founder and CEO of Bedside Health, which trains and deploys autonomous agents for Hospital at Home programs. Before Bedside, he built Singapore's national COVID-19 vaccination platform, coordinating delivery of 12 million doses across 600 clinics. His interest in Hospital at Home began with a personal experience caring for his grandmother, where he watched excellent clinical care undercut by operational coordination failures. He holds an MBA from the Stanford Graduate School of Business.
Gregory Snyder is a clinician, AAHCM Board Member and physician executive, leading technology-enabled care delivery models to improve healthcare quality and safety. He practices hospital medicine at Mass General Brigham Newton-Wellesley, and is Entrepreneur-in-Residence at the Mass General Healthcare Transformation Lab. Greg is Clinical Assistant Professor at Tufts University School of Medicine, Associate Faculty at Ariadne Labs, and Adjunct Faculty for the Institute for Healthcare Improvement. He is focused on scaling Hospital at Home programs and home-based acute care as Vice President of Clinical Strategy & Quality Improvement for Medically Home. Kamia Thakur, MD is a physician executive, healthcare innovator, and nationally recognized leader in care delivery transformation. She serves as System Medical Director of Home Recovery Care at Penn State Health, where she has led the strategic development, expansion, and operational scaling of home-based acute care programs across the health system.
Under her leadership, Penn State Health's Hospital at Home program has evolved into a high-performing, multi-site model for delivering acute hospital-level care in the home while improving patient experience, reducing unnecessary facility utilization, and supporting health system capacity management. Sina Torabi is the Co-Founder and Chief Operating Officer of Medsien, leading provider of scalable remote care management designed to foster continuity and a patient-first experience. He has focused on building physician-led care models that expand access, maximize patient engagement, and operate effectively at scale. Prior to Medsien, Sina co-founded and served as CEO of Mosaikx. His work is driven by a mission to improve access to healthcare for vulnerable patient populations through care delivery models led by physicians and optimized by technology.

During her more than two-decade career she has held numerous positions with CMS ranging from litigating attorney to member of the CMS-CDC COVID-19 Task Force. As a member of the task force, she helped draft the Hospital Without Walls waivers, and then co-led the team that wrote and implemented the CMS Acute Hospital Care at Home waiver. In addition, she also led the CDC/CMS nationwide nursing home strike force initiative to assist nursing homes that were experiencing large COVID-19 outbreaks. After leaving government in 2021, Lisa led the advocacy team at Medically Home, which was instrumental in getting Congress to extend the CMS Acute Hospital Care at Home waiver in 2022 and getting the House and Senate to introduce bills for a five-year extension of the waiver in 2024.
As the Chief Medical Officer for Mount Sinai at Home, Dr. Truong creates vision to deliver patient-centered quality care to patients in their homes. She has implemented strategies to launch, grow and integrate programs on the continuum of home-based care – Hospital at Home, Rehabilitation at Home, Palliative Care at Home and other home-care clinical services within the Mount Sinai at Home portfolio. She is passionate about addressing health equity and believes that home-based care, particularly acute at home care is well-positioned and is an important tool to advance health equity. Dr. Truong graduated from Swarthmore College in Economics and Biology, obtained her MD at Temple University Katz School of Medicine, and completed her Internal Medicine Residency at Yale-New Haven Hospital. Currently, she is an Associate Professor of Medicine at the Icahn School of Mount Sinai.
Stephanie M. Volpe, PA-C, is the Senior Surgical APP Manager & Interim APP Director for Mass General Brigham Home Hospital and serves as Senior APP Manager for the Surgical and Nocturnist clinical service lines. She brings 19 years of inpatient surgical experience in Trauma/Acute Care Surgery and extensive leadership experience in advanced practice provider operations, clinical program development, and surgical care delivery.
Stephanie has been instrumental in designing and operationalizing the Surgical Home Hospital clinical service line. Her work included developing clinical pathways, building the APP team structure, creating targeted education for frontline staff, and partnering with surgeons across the Mass General Brigham system to expand surgical care in the home setting for over 450 surgical patients to date. She has also helped develop the Nocturnist APP program to support 24-hour patient care.
In her current role, Stephanie leads APP strategy and operations across the Mass General Brigham Home Hospital, supervising two APP managers and a team of 43 APPs. She collaborates closely with clinical and operational leadership to align APP practice with organizational, strategic, financial, and operational goals. Her work is grounded in patient safety, process improvement, mentorship, and innovation in care delivery.
Dr. Ashby Wolfe is a board-certified family physician and serves as Regional Chief Medical Officer for the Centers for Medicare & Medicaid Services (CMS). Based in the CMS Seattle and San Francisco offices, Dr. Wolfe has served as a CMS Regional Chief Medical Officer since 2015, supporting the implementation of Medicare and Medicaid policy across the Western United States and the Pacific Territories (AK, ID, OR, WA, AZ, CA, HI, NV, Guam, American Samoa, and the Commonwealth of the Northern Mariana Islands). As a senior medical advisor for CMS, she also focuses on cross-cutting implementation of agency initiatives, providing clinical expertise around payment policy, quality improvement, survey operations, contracted health plans and serving as a principal liaison with the clinical community across the West and Outer Pacific. Dr. Wolfe continues to lead agency-wide work on the Acute Hospital Care at Home Initiative, as extended by Congress, following the end of the COVID-19 Public Health Emergency.
Jessica Yang, DNP, RN, ACNS-BC is a healthcare product and operations leader with nearly two decades of clinical and health technology experience. As an early team member at Medically Home, she helped launch and scale one of the first Hospital at Home programs in the U.S., leading both clinical operations and product development. As a member of Tripp Hollander Advisors, she now advises health systems on Hospital at Home strategy, workflow design, operational readiness, technology implementation, and scalable care delivery models.
Dr. Cindy Yu is the Director of the Home Hospitalist Unit at Mass General Brigham, where she oversees physicians across the Mass General Brigham Home Hospital program. In this role, she leads and supports collaboration between physicians and APPs to ensure high‑quality, team‑based care in the home setting. Dr. Yu is an Assistant Professor of Medicine at Harvard Medical School and directs the Massachusetts General Hospital Internal Medicine Resident Home Hospital elective. She practices hospital medicine at Massachusetts General Hospital and also serves as a home hospitalist with Mass General Brigham Home Hospital, combining clinical care, education, and program leadership.
Meg Zacks serves as Senior Vice President and Chief Strategy Officer for Saint Francis Health System, where she oversees enterprise strategic planning, marketing & communications, community health & government affairs, and post-acute services including hospice, home health and other home-based alternative care programs.
Prior to joining Saint Francis, Meg served as Vice President of Strategy at DispatchHealth, a national home-based acute care organization operating in more than 50 markets, where she led enterprise strategic planning and supported expansion of hospital-level care in the home. She also held strategy leadership roles at Centura Health in Denver, and CHI Health in Omaha, where her work included multi-hospital integration, service line planning, physician alignment, ambulatory network development, and capital planning across regional markets in Colorado, Kansas, Nebraska, and Iowa. Earlier in her career, she completed an administrative fellowship after earning a dual-Masters in Health Administration and Public Health Policy.
Colleen Hole serves in an advisory role, helping guide Advocate Health’s home-based care strategy, which includes scaling Hospital at Home to all markets across the Enterprise, and working to advance legislative policy, payment models and corporate partnerships that support home-based care.
Previously, Colleen served in a dual role at Atrium Health as vice president of clinical integration and chief nurse executive for Atrium Health Medical Group, responsible for non-provider clinical practice and role optimization in ambulatory care. In addition, she was responsible for strategic and operational leadership for the Atrium Health hospital at home program, which was accelerated through the COVID-19 pandemic.
Dr. Leff is Professor of Medicine at the Johns Hopkins University School of Medicine. His research focuses on novel models of care delivery for older adults with an emphasis on home and community-based models of care, issues related to multi-morbidity and serious illness, risk prediction, and quality measurement and improvement. He Co-leads the HaH Users Group and the National Home-Based Primary Care Learning Network. He is Founding Executive Editor of the Journal of Advanced Home Medicine, past -President of the American Academy of Home Care Medicine, and currently serves as a Trustee of the ABIM Foundation.
Dr David M Levine, MD MPH MA, is an internationally renowned expert in designing and implementing advanced home-based care, using digital health technology to drive health outcomes, and evaluating the quality, safety, and experience of health care. He is an Associate Professor of Medicine at Harvard Medical School, clinical director of research and development for Mass General Brigham Health Care at Home, and co-director of the Harvard General Internal Medicine Fellowship at Brigham and Women’s Hospital. He founded Brigham’s home hospital program and co-founded and co-chairs the Hospital at Home Users Group, a collaborative of home hospital programs throughout the US and Canada. He is core faculty at Ariadne Labs, a center for health systems innovation at the Harvard School of Public Health and Brigham.
Michael J. Maniaci, M.D. is the Virtual Health & Home-Based Care Clinical Operations Associate Vice President for Advocate Health. In this role, he oversees the strategic and operational development of virtual health and home-based care programs, driving innovation in high-acuity care delivery models that extend clinical services beyond traditional hospital settings.
Previously, Dr. Maniaci held multiple leadership roles at Mayo Clinic, including Enterprise Physician Lead for the Advanced Care at Home program, Medical Director of Consumer Digital Product in the Center for Digital Health, and Medical Director of Mayo Clinic Hospital in Florida. His other leadership positions included Chair of the Division of Hospital Internal Medicine and Associate Medical Director of the Mayo Clinic Multidisciplinary Simulation Center in Florida.
Ila Mapp, DNP, RN, CENP, CCRN has been enhancing her nursing craft for over 20 years through travel nursing, bedside patient care, and leadership. In 2008, Ila joined the UNC Health organization advancing from clinical nurse to Nurse Manager. She has held a leadership roles within UNC Medical Center and UNC Health since 2013 with her specialty in critical care managing four specialty teams: Rapid Response, ICU Transport, Venous Access Team, and CVAD Liaison. Ila now serves as Director for Advanced Care at Home – an innovative utilization of technology providing acute care for patients in their homes. This model of healthcare incorporates the collaboration of internal and external community stakeholders inclusive of Paramedics and Home Health Nursing working together with the common goal of patient-centered care for the Triangle of North Carolina.
Constantinos (Taki) Michaelidis, MD, MBA, MS cares for patients as a Virtual Home Hospitalist in the UMass Memorial Medical Center (UMMMC) Home Hospital program where he is Medical Director and faculty in the Program in Digital Medicine at the University of Massachusetts Medical School. Prior to joining UMMMC, he was Senior Medical Director in the Atrius Health home hospital program. There, he supported local and national home hospital program development and designed and scaled new acute and post-acute home-based care models for a predominantly frail, medically complex and home-bound population in eastern Massachusetts.
Dr. Richard Rothman is a Regional Chief Medical Operations Officer for Cleveland Clinic Florida and the Vice Chief for Integrated Hospital Care Institute for Cleveland Clinic. In his role, he oversees Cleveland Clinic Hospital Care at Home. Dr. Rothman graduated from St. George’s University School of Medicine in 2009. He completed a post-doctoral research fellowship in Boston, Ma, at the Massachusetts General Hospital and completed both his internship and residency at the Lahey Clinic, also in Massachusetts. He received his diplomate in Internal Medicine from the American Board of Internal Medicine. He is active member of the American College of Physicians and Society of Hospital Medicine.
Hemali Sudhalkar, MD, MPH is an Internal Medicine Hospitalist and Palliative Care physician at The Permanente Medical Group in San Jose, CA. She serves as the Regional Medical Director of Hospital Quality and Care Experience for the 21 medical centers within Kaiser Permanente’s Northern California region. Dr. Sudhalkar received her medical education in India and earned a Master of Public Health degree in Epidemiology from Emory University. She completed her Internal Medicine Residency at Kaiser Permanente in Santa Clara, CA. Dr. Sudhalkar is dedicated to ensuring that every patient receives the highest quality care, at the right place and time, with a holistic focus on the needs of both the patient and their family. She has a Six Sigma Black Belt in process improvement and actively applies lean methodologies to enhance care delivery.
