Senior Director, SHC at Home Program Stanford Health Care
Sophia Loo is the Senior Director of the SHC at Home Program at Stanford Health Care, bringing more than 25 years of healthcare leadership experience across transitional care, neuroscience, cardiovascular services, and ambulatory operations. She has successfully led multidisciplinary teams, managed multimillion-dollar budgets, and driven operational excellence across inpatient, outpatient, and home-based care settings. Under her leadership, SHC at Home has served more than 1,000 patients, achieved an average daily census of 45 patients, and generated approximately 0.5 to 1.0 bed-days saved per case. Sophia is passionate about advancing patient-centered, data-driven care, fostering strategic partnerships, and implementing innovative care models that improve quality, access, efficiency, and patient experience across the health system.
