Dr. Jared Conley MD PhD MPH is an Assistant Professor at Harvard Medical School and an emergency physician at Massachusetts General Hospital (MGH). He serves as the Associate Director of the MGH Healthcare Transformation Lab, where he leads a team of clinicians and engineers to enhance the quality and affordability of acute healthcare through technology and innovation.
He additionally serves as an advisor to MGH’s Home Hospital program and as the Medical Advisor for the World Hospital at Home Community. His digital health work is broad but is particularly focused on new, tech-enabled models of care, including hospital at home (HaH). He was recently asked to chair the first-ever HaH technology forum in March 2023.
Thomas Kiesau is a Senior Partner with Chartis, the firm’s Chief Innovation Officer, and leader of the Digital Practice, the firm’s business unit dedicated to digital transformation planning and execution across the healthcare arena. Prior to assuming leadership of the Digital Practice, he led the firm’s Strategy Practice. With more than 20 years of experience, Mr. Kiesau has served as an advisor to many of the nation’s leading children’s hospitals, academic health centers, faculty practice groups, integrated health systems and community hospitals. He directs consulting engagements principally in the area of digital transformation, including digitally-enabled care model transformation, home as a site of care, digital consumer experience and digital operating model design and implementation.

He is a practicing general internist and clinician-investigator at Brigham & Women’s Hospital and Harvard Medical School, where he founded and co-directs Brigham’s home hospital program and co-directs the general internal medicine fellowship. He co-founded and co-chairs the Hospital at Home Users Group, a collaborative of home hospital programs throughout the US and Canada. He is core faculty at Ariadne Labs, a center for health systems innovation at the Harvard School of Public Health and Brigham, where he studies rural home hospital and scaling home hospital.

Danielle has been working in healthcare since 2009. She entered the Hospital at Home space in 2020, helping launch the Mayo Clinic Advanced Care at Home program, while employed at Medically Home. At Medically Home Danielle also worked with the Cleveland Clinic, Kaiser Permanente, and Covenant Health HaH programs. In 2024 she left Medically Home and joined Baptist Health based in Jacksonville Florida where she serves as the Operations Director responsible for building, launching, and overseeing Baptist Hospital at Home. She has her bachelor’s degree from Penn State and is currently finishing her MBA from Eastern University.
Dr. O’Shea is interested in applying his training in clinical medicine and business administration to build and refine systems of care for hospitalized adults and children. In addition to serving as a hospitalist, he works as a physician advisor to support clinical operations across UNC Hospitals.
Dr. Margaret Paulson is an Assistant Professor of Medicine at the Mayo Clinic College of Medicine and Science. She has experience in patient care across the spectrum of Internal Medicine. She currently practices as a rural hospitalist in the Mayo Clinic Health System in the Northwest Wisconsin (NWWI) region and leads the Home Health team. In 2019, she became the Medical Director for NWWI in the exciting establishment of Mayo Clinic’s Advanced Care at Home (ACH) program, an innovative platform offering hospital-level care in the home. The launch of ACH allowed Dr. Paulson to call upon her experience in outpatient and inpatient Internal Medicine to expand high quality telemedicine in transformational ways for Mayo Clinic.
Eliza “Pippa” Shulman, DO, MPH, is one of the world’s foremost authorities on providing advanced care in the home. She joined Dispatch Health’s leadership team during the company’s merger with Medically Home, where she served as Chief Medical Officer and Chief Strategy Officer. As the clinical architect behind many of Medically Home’s most innovative models, Dr. Shulman helped shape what is now part of Dispatch Health’s approach to high-acuity care in the home.
Gregory Snyder is a clinician and physician executive, leading technology-enabled care delivery models to improve healthcare quality and safety. He practices hospital medicine at Mass General Brigham Newton-Wellesley, and is Entrepreneur-in-Residence at the Mass General Healthcare Transformation Lab. Greg is Clinical Assistant Professor at Tufts University School of Medicine, Associate Faculty at Ariadne Labs, and Adjunct Faculty for the Institute for Healthcare Improvement. He is focused on scaling Hospital at Home programs and home-based acute care as Vice President of Clinical Strategy & Quality Improvement for Medically Home. Greg has partnered with diverse healthcare technology ventures to improve healthcare quality, safety, value, and experience.
Lisa is a nationally recognized expert on hospital at home law, policy and advocacy. During her more than two-decade career she has held numerous positions with CMS ranging from litigating attorney to member of the CMS-CDC COVID-19 Task Force. As a member of the task force, she helped draft the Hospital Without Walls waivers, and then co-led the team that wrote and implemented the CMS Acute Hospital Care at Home waiver. In addition, she also led the CDC/CMS nationwide nursing home strike force initiative to assist nursing homes that were experiencing large COVID-19 outbreaks. After leaving government in 2021, Lisa led the advocacy team at Medically Home, which was instrumental in getting Congress to extend the CMS Acute Hospital Care at Home waiver in 2022 and getting the House and Senate to introduce bills for a five-year extension of the waiver in 2024.
As the Chief Medical Officer for Mount Sinai at Home, Dr. Truong creates vision to deliver patient-centered quality care to patients in their homes. She has implemented strategies to launch, grow and integrate programs on the continuum of home-based care – Hospital at Home, Rehabilitation at Home, Palliative Care at Home and other home-care clinical services within the Mount Sinai at Home portfolio. She is passionate about addressing health equity and believes that home-based care, particularly acute at home care is well-positioned and is an important tool to advance health equity. Dr. Truong graduated from Swarthmore College in Economics and Biology, obtained her MD at Temple University Katz School of Medicine, and completed her Internal Medicine Residency at Yale-New Haven Hospital. Currently, she is an Associate Professor of Medicine at the Icahn School of Mount Sinai.
Colleen Hole serves in an advisory role, helping guide Advocate Health’s home-based care strategy, which includes scaling Hospital at Home to all markets across the Enterprise, and working to advance legislative policy, payment models and corporate partnerships that support home-based care.
Previously, Colleen served in a dual role at Atrium Health as vice president of clinical integration and chief nurse executive for Atrium Health Medical Group, responsible for non-provider clinical practice and role optimization in ambulatory care. In addition, she was responsible for strategic and operational leadership for the Atrium Health hospital at home program, which was accelerated through the COVID-19 pandemic.
Dr. Leff is Professor of Medicine at the Johns Hopkins University School of Medicine. His research focuses on novel models of care delivery for older adults with an emphasis on home and community-based models of care, issues related to multi-morbidity and serious illness, risk prediction, and quality measurement and improvement. He Co-leads the HaH Users Group and the National Home-Based Primary Care Learning Network. He is Founding Executive Editor of the Journal of Advanced Home Medicine, past -President of the American Academy of Home Care Medicine, and currently serves as a Trustee of the ABIM Foundation.

Constantinos (Taki) Michaelidis, MD, MBA, MS cares for patients as a Virtual Home Hospitalist in the UMass Memorial Medical Center (UMMMC) Home Hospital program where he is Medical Director and faculty in the Program in Digital Medicine at the University of Massachusetts Medical School. Prior to joining UMMMC, he was Senior Medical Director in the Atrius Health home hospital program. There, he supported local and national home hospital program development and designed and scaled new acute and post-acute home-based care models for a predominantly frail, medically complex and home-bound population in eastern Massachusetts.
Dr. Richard Rothman is a Regional Chief Medical Operations Officer for Cleveland Clinic Florida and the Vice Chief for Integrated Hospital Care Institute for Cleveland Clinic. In his role, he oversees Cleveland Clinic Hospital Care at Home. Dr. Rothman graduated from St. George’s University School of Medicine in 2009. He completed a post-doctoral research fellowship in Boston, Ma, at the Massachusetts General Hospital and completed both his internship and residency at the Lahey Clinic, also in Massachusetts. He received his diplomate in Internal Medicine from the American Board of Internal Medicine. He is active member of the American College of Physicians and Society of Hospital Medicine.
Hemali Sudhalkar, MD, MPH is an Internal Medicine Hospitalist and Palliative Care physician at The Permanente Medical Group in San Jose, CA. She serves as the Regional Medical Director of Hospital Quality and Care Experience for the 21 medical centers within Kaiser Permanente’s Northern California region. Dr. Sudhalkar received her medical education in India and earned a Master of Public Health degree in Epidemiology from Emory University. She completed her Internal Medicine Residency at Kaiser Permanente in Santa Clara, CA. Dr. Sudhalkar is dedicated to ensuring that every patient receives the highest quality care, at the right place and time, with a holistic focus on the needs of both the patient and their family. She has a Six Sigma Black Belt in process improvement and actively applies lean methodologies to enhance care delivery.
