Nolan Hoffer

Vice President of Operations Home and Community-Based Services, St. Lukes Health

Nolan Hoffer serves as Vice President of Operations for Home & Community-Based Services at St. Luke’s Health System, where he leads the scaling of Hospital at Home, virtual care, and post-acute services across the continuum.

With more than 35 years of executive leadership experience, Nolan has overseen operations across the full post-acute spectrum and has deep expertise in integrating care delivery models to improve access, quality, and patient experience. He has been instrumental in advancing innovative care models, including SNFist and Transitions of Care programs, and the development and expansion of St. Luke’s Hospital at Home.

Since joining St. Luke’s in 2014, he has played a key role in system integration, including the incorporation of Idaho Elks Rehabilitation Hospital, and now leads a diverse portfolio spanning therapy services, home health, hospice, and system chaplaincy.
Nolan is known for leveraging quality as the foundation for scalable, patient-centered care.
