Dr. Stephanie Murphy is an Internal Medicine Hospitalist at Atrium Health in Charlotte, North Carolina and a Physician Implementation and Transformation Lead at Medically Home Inc. in Boston, MA. Stephanie has been working in innovative care delivery models since 2015 when she became Medical Director and clinical creator of the Atrium Health Transition Services program. This clinic focused on high risk medically complex patients coming out of the hospital and employed a multidisciplinary approach to their medical care and readmission risk reduction. In conjunction with the development of the Transition Services program, Stephanie became co-medical director for Atrium Health’s Mobile Integrated Health program which quickly became the largest MIH program in the country. In 2020, she became Medical Director and clinical creator of the Atrium Health Hospital at Home.
