Tom Hackford, MD has worked as an Adult Hospitalist for over 20 years and joined The Permanente Medical Group in 2010 practicing out of Richmond, CA. He attended medical school at New York University and completed his residency training at University of California, Los Angeles. After joining the Kaiser Permanente team, he served in various leadership roles in his local Hospital Medicine Department as well as Resource Management Chief of the Kaiser Permanente Richmond Medical Center. Tom joined the Kaiser Permanente Northern California Advanced Care at Home (ACAH) program in 2022 where he has served as the Physician Quality and Safety Lead. His focus since joining ACAH has been to ensure consistent, high-quality care in a rapidly evolving environment.
